Warranty Claim Form
To make a warranty claim:
- Complete the Service Request Form below.
- The claim will then be evaluated.
- If the part is not covered under warranty, you will be notified.
- If the part is covered under warranty, the order will be processed and a confirmation email will be sent.
- If the part requires installation, it will be sent to the service technician in the area and you will be contacted by an ergoCentric Representative.
Every ergoCentric chair is assigned a serial number. The serial number is saved in our database with all of the specifications and information pertaining to your chair.
The serial number can be found under the seat pan of your chair on the mechanism.
